Sync error and disconnect states for Stealth AI

Error and disconnect states for integrations in Stealth AI.

When a sync fails, the integration details modal leads with a red error that says what to do: check the settings, or disconnect and reconnect, and reconnecting won’t lose any data. The status changes to Unable to sync. Disconnecting asks for confirmation and warns that the data will be lost and the action can’t be undone. A toast covers the moments when you’re elsewhere in the app, with a link to the integrations page.
